 HIV Primary Care Fellowship
Purpose
As the epidemic of human immunodeficiency virus (HIV) continues, treatment and care of this condition in the United States is transitioning toward a chronic disease model. It is becoming increasingly important for primary care physicians to be current with and comfortable in providing basic care to patients infected with HIV.

The Family Medicine Residency of Idaho (FMRI) HIV Primary Care Fellowship is a 12-month post-residency training opportunity that provides a suitable environment for the Family Medicine physician to gain additional experience providing comprehensive care to HIV-positive patients, including management of antiretroviral therapy and opportunistic infections. This fellowship will also place specific emphasis on provision of care to the underserved and rural HIV-positive populations.

The HIV Services Clinic at FMRI is a federal, Ryan White grant recipient clinic providing comprehensive interdisciplinary medical care to persons living with HIV/AIDS

Goals
  • Gain extensive experience providing primary medical care for HIV-positive patients. This will include gynecologic management for female patients, as well as involvement in occasional obstetric and pediatric cases.
  • Become proficient in the independent management of antiretroviral therapy regimens and opportunistic infection care.
  • Participate in the medical co-management of all HIV-positive patients admitted to either tertiary care hospital system in Boise (St. Alphonsus and St. Luke’s Regional Medical Centers) by directly collaborating with infectious disease consultants.
  • Publish one research or review article focusing on HIV primary care (target audience: family medicine physicians; target journal: AFP).
  • Maintain family medicine clinical skills by conducting a general family medicine clinic and participating as junior faculty in the Family Medicine Residency of Idaho.
 Educational Objectives
  • 10% general family medicine, 70% HIV primary care, 10% didactic, 10 % research and teaching.
  • Evaluation of fellow by HSC clinical director and FMRI director or co-directors.
  • Participate in NW AIDS Education Training Center endeavors and opportunities, including attending the Pacific/NW AETC Asilomar faculty development conference.
  • Participate in and assist ongoing HIV-related community health efforts (i.e. AETC presentations, interactive seminars with ALPHA, World AIDS day activities, etc.).
  • Assist in development of disease management and patient education groups for HIV positive patients facilitated by members of the HSC team and patients.
  •  Provide Idaho HIV update or other pertinent HIV primary care topic to community providers at medical grand rounds (Ada County Medical Educational Consortium).
  • Provide 2-3 HIV-related noon conference presentations to FMRI residents/faculty and monthly HIV teaching topics to the residency inpatient medicine teams.
  • Two externships (2-3 days) with HIV care providers at UW Madison Clinic, Seattle, Washington and Multnomah County HIV Services Center Portland, Oregon (or other NW AETC approved clinical sites).
  • Attend annual Ryan White Care Act clinical conference and/or International AIDS Society conferences.

Clinical Objectives 

  1. Six half-day clinic slots per week dedicated to outpatient HIV care.
    • Four clinics precepted by ID specialists, two urgent care clinics.
    • The fellow will see the majority of patients in precepted clinics and present to attending ID specialist.
  2. One or two half-day clinics per week dedicated to general primary care clinical duties.
  3. One half-day per week dedicated to chart review, laboratory review, primary medical issues, referrals, etc. for all patients at HSC.
  4. One half-day per week dedicated to prevention case management and one-on-one behavior modification intervention at HSC.
  5. Two days per month dedicated to assisting the HSC social worker staff with longitudinal efforts; i.e. ongoing development of free, rapid testing programs in Boise, assessing HIV awareness and risk factors in Latino communities in Boise, Nampa and Caldwell, etc.

Salary: competitive.
Vacation: 15 days per year.

NW AETC support for conference attendance and clinical preceptorships.
Licenses and Dues: board of medicine, DEA, board of pharmacy, AAHIVM accreditation.

Applicant must be board certified, or eligible for board certification, in Family Medicine.
